[CS Bureau] First Draft of NA and Europe Region Report

Attached is the first draft of my report for the North American and 
European Region.

As it's more or less a copy of the oral report which I gave @ Cape 
Town, I will over the next week clean up the text and include 
comments that have been sent to me from my EU and North American 
collegues.
