[CS Bureau] PrepCom2 Registration Form

Dear Renata,

I've taken a look at this form, and find it a little confusing. What's 
the difference between the contact person and the person legally 
responsible? Where do we actually insert the name of the nominee to 
participate in WSIS Prepcom 2? Is this form to be completed in addition 
to the one that Adam informed us about for CS fellowships?

Please clarify. Much thanks in advance for your advice.
