[CS Bureau] addresses

Dear Divina,

Just to clarify. During the WSIS PrepCom, we decided that it was 
necessary to update the address file. A list was handed out around the 
table and people filled in their contact details by hand. I took that 
(not very complete list, not everyone was at the meeting) to complete 
the address file we already have for the bureau. 

I suggest that the easiest way to get a really up-to-date list is for 
everyone to fill in the form that Robert made. He just resent the link 
yesterday.
