[Ftx-trainers] proposed FTX evaluation form and questions

Hi Manal, Maru and all,
It really depends if participants will have time to get online, remain
online, and respond to the questions. Can they upload their responses
after completing the form offline? I think it might be easier to
administer it through e-mail and offline. Not sure about this right now.
What do others feel? It might be good to put these already on the FTX
website, but I can also just encode the responses. Maybe we can provide
both methods of an online form and through e-mail, so that we will
already have the data on record.
IMPORTANT: The only thing then I would need trainers to make sure is
that everyone completes and submits their responses to the evaluation
before they leave for lunch on Day 3.
I'm giving everyone until 3 pm today (Day 1) to let me know if there are
gaps in the questions posed but you have to develop the question(s) you
want asked in addition to these and I will then finalise the form, or if
there's anything repetitive/overlaps, or if there are very specific
evaluation questions you want asked of your track participants.
So hopefully, by this evening, we can send out the questions to
participants.
